Appointments
We schedule office visits by appointment only.
To make, change, or cancel an appointment, please call our office (216) 520-5969 during our regular front office hours.
To better serve your needs, we recommend making follow-up appointments while in the office, either before or after you see your clinician. If you need a specific appointment time, it is often helpful to schedule several appointments in advance.
We try to see all patients on time and request that you extend the same courtesy to us by arriving before your scheduled time.
There may be times when your clinician is running late due to an emergency. If this occurs, we will make every attempt to let you know as quickly as possible.
*Please note: If you cancel your appointment less than 24 hours in advance, there may be a cancellation fee applied to your account.
**Also note: Affiliates reserves the right to not reschedule first appointments that are broken without such notice.
If you call our office and feel that you have an urgent matter which cannot wait, tell the person who answers the call that the matter is urgent. We will make every attempt to offer you an appointment as quickly as possible.
New Patients
To schedule your first appointment, start by calling our Intake & Scheduling Department at (216) 520-5969. New patient appointments are generally available in less than 2 or 3 weeks.
Because our goal is to offer you the best match between your clinical needs, geography, insurance, preferences, and schedule, it will help to have all of your information available when you speak with our intake and scheduling staff, including:
- Your insurance card
- Drivers license
- Any medications you’re currently taking
We request that you arrive fifteen minutes early for your first appointment to complete your paperwork.

Although we see patients evenings and weekends, our front office hours are as follows:
Monday: 9am – 5pm
Tuesday: 9am – 5pm
Wednesday: 9am – 5pm
Thursday: 9am – 5pm
Friday: 9am – 12pm

Refills
You will generally find that your doctor times your refills to be consistent with your needed follow-up visits. However, there will be times when you will need to contact our office to request a prescription refill.
Please call the doctor’s psychiatric assistant or use the online refill request form to request a refill of any medication.
Before the doctor can refill your medication, you will need to have a follow-up appointment scheduled.
Because your doctor will need to have your medical record available, please contact our office during regular business hours.
Whether you request a refill online or by phone, we will require your name and phone number, the name, dosage and directions on the prescription bottle and the date of your last visit with your doctor.
*Note: Our psychiatrists are unable to refill prescriptions written by physicians outside of Affiliates in Behavioral Health.
