Please remember that the patient is financially responsible for services rendered, regardless of insurance coverage. You should check your health insurance coverage to verify your provider is in your insurance network plan. 

Our office will file insurance claims with your primary carrier. If your insurance company requires a referral or authorization, this must be obtained by you PRIOR to your first visit. If you fail to obtain the referral or if you do not follow your insurance company’s policy, you are responsible for payment for services rendered. 

Due to insurance company rules, co-payments and deductibles must be paid at the time of service. Payments not made at this time are subject to administrative fee surcharges and interest.
