Please remember that the patient is financially responsible for services rendered regardless of insurance coverage. You may check your health insurance coverage by clicking here. To verify your provider is in your insurance netwrk plan, please verify with your insurance company's member services department. Our office will file insurance claims with your primary carrier. If your insurance company requires a referral or authorization, this must be obtained by you PRIOR to your first visit. If you fail to obtain the referral or if you do not follow your insurance companies policy, you are responsible for payment for services rendered. Due to insurance company rules, copayments and deductibles must be paid at the time of service. Payments not made at this time are subject to administrative fee surcharges and interest.
